Accessing Services

Residents who are frail, aged, have a disability or are carers of these people, are eligible for an assessment. Eligibility for an assessment does not ensure services will be provided as we prioritise services to those most in need. Qualified Assessment Officers visit you at home and work with you to explore what you can do for yourself and what you need help with to stay safe and well at home.

An assessment enables individual needs to be recognised and met through Council and other health services. Together we develop a Care Plan which aims to help you maintain and strengthen your ability to remain living independently at home and in your community. These plans complement your other supports such as family, friends, local community groups, your doctor and other health professionals. Specific plans will be developed for those receiving personal care or respite care.

Any support set up will depend on what is important to you and what is available. Sometimes we connect you to other services or groups, or our support may be short term until you get back on your feet.
