Aged and Disability Services
Council’s Home and Community Care (HACC) service provides a range of services designed to support you, or the person you care for, to stay active, independent and living at home for as long as possible. Helping people stay active, involved and doing as much for themselves as possible is proven to help people stay more independent with a greater sense of well-being. This is the approach we take in working with you.
Frail older people, people with disability and carers of these people who are residents of Bass Coast, are eligible for an assessment. Assessment enables individual needs to be recognised and met through Council or other health services.
Council recognises that we are all diverse and aims to support those who may require additional assistance to access our services.
